The Community Care Network (CCN) serves as the direct link between the VA and community providers like Family Resource Home Care. The VA determines a veteran’s enrollment eligibility for home care benefits through the CCN program. Upon acceptance of enrollment in CCN, a referral is generated to a VA case manager through the Homemaker/Home Health Aide Program (HHHA), where an evaluation with the veteran will determine how many home care hours are allocated and what the care plan should be. This program enables veterans to receive care from community providers in collaboration with the VA to ensure timely and accessible support. The CCN aims to expand veterans’ access to high-quality healthcare services, including in-home care, by integrating community resources with VA benefits.
